<p>
 One of the most common combinations, a triple therapy with a combination of intravenous PMT, oral CPA, and alternate-day prednisone therapy, was first described by Griswold. Large doses, 30 mg/kg methylprednisolone up to a maximum of 1 g, were infused intravenously three times per week for 2 weeks (six doses) and then weekly for at least 4–6 weeks.
</p>
<p>
 Decreasing the frequency of the PMT dose to every other week and monthly occurred when UProt/UCreat levels decreased to near normal levels. For patients who did not respond to pulse intravenous corticosteroid therapy (PICT) alone, oral CPA at 2 mg/kg/day was prescribed for 75–90 days. Alternate-day oral corticosteroid dosing was decreased relative to the patient’s response.
</p>
<p>
 Nitrogen mustard therapy (mechlorethamine) and vincristine There have been no RCTs of mechlorethamine. Mechlorethamine therapy with corticosteroids or adrenocorticotropic hormone was advocated as an effective treatment for SRNS from the 1950s to the 1980s.
</p>
<p>
 All patients achieved partial or complete remission in the study conducted by Fine, in which mechlorethamine was infused intravenously for four consecutive days. A subsequent study by Armugan did not report encouraging results.
</p>
<p>
 None of the seven patients achieved a complete remission. Vomiting was a common adverse effect of mechlorethamine therapy. although primarily used as an antineoplastic drug in the treatment of malignancies, there have been two small cohort studies of vincristine treatment in children with SRNS. Response rates to therapy, defined as combined complete and partial remissions, were 38% and 43%.  
</p>
<p>
 Both studies concluded that vincristine was relatively ineffective in inducing remission. However, patient selection for both studies appeared to be biased towards unresponsiveness to any therapy. There is insufficient evidence to make a recommendation regarding vincristine therapy.  
</p>
<p>
 In the six triple therapy studies (150 patients), 50–75% of patients achieved a complete or partial remission. African American patients may be less responsive to triple therapy than other ethnic groups, and outcomes in patients who have failed treatment with calcineurin inhibitors have been disappointing. A long-term analysis of FSGS patients by Tune reported that 9% of patients progressed to ESRD.
</p>
<p>

 Waldo conducted a prospective cohort study in which PMT and CsA therapy was combined in 10 children with FSGS, 80% of whom achieved remission. Four of the patients were African American, and all four responded to therapy. Yorgin also reported the use of combined PMT, angiotensin converting enzyme inhibitor (ACEi), and CsA therapy in a subset of study subjects. CsA therapy was used to maintain remission after it had been achieved by PMT.
</p>
<p>
 Kano reported the successful use of combination therapy with PMT, intravenous immunoglobulin and a statin, with 77% achieving a complete or partial remission.The study by Mori prospectively evaluated the use of heparin therapy and PMT administered for three consecutive days. The combination therapy was given 14 times over 2 years.
</p>
<p>
 The patient group consisted of children who were expected to have a poor prognosis due to their resistance to CPA or CsA, and 60% achieved a partial or complete remission.El-Reshaid reported a 100% remission rate in 21 children with SRNS in a sequential protocol consisting of a calcineurin inhibitor followed by the addition of MMF and then by monthly intravenous CPA for three consecutive months. All patients required antihypertensive therapy after starting calcineurin inhibitor therapy.
</p>
<p>
 The use of combination therapy, consisting of two or three different medications, has been advocated by many investigators. There have been at least 10 studies with a total of 204 patients treated with combination therapy, but no RCT has been conducted. The mean complete remission rate for all combination therapies is 58%. Nearly all patients with MCD respond to combination therapy,whereas only 47% of FSGS patients achieve a complete remission.

<p><a  href='http://curesfibromyalgia.com/wp-content/plugins/wp-affiliate-pro.php?id=2' target="_blank">fibromyalgia</a> is a muscle and joint disease that affects approximately two to four percent of the population, <a  href='http://curesfibromyalgia.com/wp-content/plugins/wp-affiliate-pro.php?id=2' target="_blank">fibromyalgia</a> is predominantly diagnosed in women between 20 and 50, although it can affect men and even children. It’s estimated some twenty million Americans suffer from this disorder. Fibromyalgia is said to be the most common arthritis related condition after osteoarthritis.</p>
<p> The cause of fibromyalgia is not clearly understood. Some researchers theorize decreased levels of serotonin contribute, while others think there’s a genetic or environmental component. Some researchers feel there is even a link between childhood abuse and the propensity to develop fibromyalgia. Many practicing physicians don’t even recognize fibromyalgia as a true diagnosis but most rheumatologists do. It can be difficult to find a medical practitioner sympathetic to the condition and familiar with its treatment.</p>
<p>Diagnosing Fibromyalgia is very difficult. It’s been called the great imitator because it’s often misdiagnosed as arthritis, Lyme disease and other disorders. The diagnosis is primarily made by ruling out other diseases and by symptoms reported by the patient.</p>
<p>Symptoms include muscle pain and stiffness lasting more than three months, sore throat, swollen glands and painful tissue with normal blood work, poor sleep and fatigue in the mornings. In addition, there are specific trigger points that are generally inflamed and tender. The symptoms of fibromyalgia can be aggravated by stress, cold, humidity, over exertion, lack of sleep and infections. Allergies to certain foods and things may also serve as triggers to cause an acute flare up of symptoms. Some research has suggested diet changes; including switching to a vegetarian lifestyle may be very helpful. Pain is the overriding symptom of fibromyalgia and as in many chronic pain disorders the risk of developing secondary addiction problems is high. A pain management doctor may be recommended. It may even be in the fibromyalgia suffers best interest to ask for a pain management referral as the FDA frowns on family doctors and internist prescribing chronic pain medication.</p>
<p>Treatment is a combination of modalities. Therapy to learn better coping mechanisms and decrease stress, finding medications and practices that improve sleep, changes in diet, gentle exercise and physical therapy have all proved to be of benefit. Various medications including serotonin reuptake inhibitors such Prozac, Tricyclic antidepressants such as Elavil, muscle relaxants such as Flexeril, non steroidal anti inflammatory drugs, and Cymbalta. Some physicians will order trigger point injections with some success in pain relief.</p>
<p>Fibromyalgia is a difficult condition both to diagnose and treat. Research is continuing in an attempt to develop better diagnostic and treatment options. At the present time treatment has only about a fifty percent success rate, however this can be improved by a combination approach that includes all the treatment modalities available.</p>

 Cramps most commonly afflict cyclists who work their muscles to a point of exhaustion.  They can occur when you have an electrolyte imbalance, a low blood glucose level, or inadequacies in hydration, conditioning or flexibility.
</p>
<p>
 The best training tip to address this problem is to do uphill intervals.  Attack a hill for 20-30 second, then recover briefly before doing it again.  This will improve our muscle strength and anaerobic endurance.  In addition, you should have your riding position checked at a qualified shop, since bike fit affects your muscles.  Also, periodically stand and stretch your quadriceps and calves while riding.
</p>
<p>
 As far as dietary recommendations, it is critical that you avoid becoming dehydrated.  Water is ideal, but if the ride is longer than one hour, you may want to add a sports drink.  In hot weather, increase your fluid intake beyond the normal guideline of one standard water bottle.
</p>
<p>
 During a ride, make sure to eat high carbohydrate foods to maintain your blood glucose level, and drink plenty of fluid to avoid dehydration.  After riding, rehydrate properly.  Have at least 3 glasses of water or energy drink.  Excess fluid poses no danger because it’s removed by body as urine.  Don’t drink beer or wine until you’re rehydrated.  Alcohol is a diuretic, causing fluid loss.  Energy drinks can help reduce cramping by providing electrolytes.  These are minerals (sodium, chloride, potassium) that carry an electrical charge that’s needed for muscle contractions and the maintenance of fluid levels.  Electrolytes are one reason potassium-rich bananas are often suggested as a cycling food. Maintain a diet rich in electrolytes.  These substances are essential for muscle contraction during exercise, and low levels have been associated with cramping.  For calcium, eat non-fat dairy products, tofu, dark green vegetables, and sardines.  For potassium, include fruits, juices, and vegetables, particularly squash, potatoes, beans, and mushrooms.  For magnesium, eat whole grains, nuts, dairy products, green vegetables, and legumes.
</p>
<p>
 In a study conducted at the Hotter ‘n Hell Hundred in Texas, the medical staff found that the difference between crampers and non-crampers was magnesium.  The riders who cramped had less of it.  To determine if this deficiency is causing your problem, get a blood test either after a ride or after your cramping begins.
</p>
<p>
 When a post-ride cramp does occur, immediately stretch the hamstring muscles by straightening your leg and bringing the toes of your foot toward your knee.  At the same time reach down and massage the area.  Believe it or not, a hard pinch on the upper lip may also provide immediate relief, although no one is certain why.  Some experts speculate that it causes an alteration in the neurotransmission from the brain to the muscle.  Whatever the reason, it often works.

By the end of the study, many patients reported up to a 50% reduction in pain after treatment with Lyrica when compared with the placebo group. Lyrica also seemed to have positive effects on the sleep patterns of fibromyalgia sufferers and helped to reduce daytime fatigue. The most frequent side effects associated with the use of Lyrica were dizziness and excessive sleepiness.</p>

The disease can make a person nearly disabled, but can also be mild or almost disappear over time.</p>
<p><strong>THE SYMPTOMS OF <a  href='http://curesfibromyalgia.com/wp-content/plugins/wp-affiliate-pro.php?id=2' target="_blank">fibromyalgia</a></strong></p>
<p>By fibromyalgia there are tender points in muscles, tendons and ligaments around the body.  The points are especially common around the joints. Tender areas in the mid parts of the muscles are fewer and with lighter pain. The points give a constant aching and a sharper pain by pressure. However, muscular work and exercise do not necessarily increase the pain. When the muscles are warmed up and exercised, the pain may disappear or decrease for some time.</p>
<p>The neck, shoulder area, hips, knee surroundings, jaw joint areas, head, face, and eye surroundings are especially often affected. When the neck and head area is affected, the pain often takes the form of a common headache.</p>

<p>A decreased activity of glands producing saliva, sebum and tears.</p>
<p><strong>LIFESTYLE MEASURES AND EXERCISE TO TREAT FIBROMYALGIA</strong></p>
<p>It is difficult to find a treatment that completely cures the disease, but a skillful combination of lifestyle measures and sometimes also medical treatment can take away most symptoms.</p>
<p>The symptoms of fibromyalgia can make it difficult to attain an active lifestyle. Yet, to do work, sport activities and hobby activities will decrease the symptoms and the consequences of the disease.</p>
<p>Regular training will greatly decrease the symptoms of the disease. The training should engage all the muscles of the body and also include exercises that improve the condition.</p>

<a  href='http://curesfibromyalgia.com/wp-content/plugins/wp-affiliate-pro.php?id=2' target="_blank">fibromyalgia</a> (FM) is a human disorder classified by the presence of chronic widespread pain and tactile allodynia.[1] While the criteria for such an entity have not yet been thoroughly developed, the recognition that fibromyalgia involves more than just pain has led to the frequent use of the term &#8220;fibromyalgia syndrome&#8221;. It is not contagious, and recent studies suggest that people with fibromyalgia may be genetically predisposed.[2] The disorder is not directly life-threatening.</p>
<p>Do you have pain from head to toe? Are you tossing and turning throughout the night, unable to sleep? Do you wake up to pain and a foggy brain in the morning? These are common symptoms experienced by fibromyalgia syndrome (FMS) and chronic fatigue syndrome (CFS) patients.The most common sites of pain include the neck, back, shoulders, pelvic girdle, and hands, but any body part can be affected. Fibromyalgia patients experience a range of symptoms of varying intensities that wax and wane over time.</p>
<p>Signs and symptoms<br />
Widespread pain. Fibromyalgia is characterized by pain in specific areas of your body when pressure is applied, including the back of your head, upper back and neck, upper chest, elbows, hips and knees. The pain generally persists for months at a time and is often accompanied by stiffnessYou may have some degree of constant pain, but the pain may get worse in response to activity, stress, weather changes and other factors. You may have a deep ache or a burning pain. You may have muscle tightening or spasms. Many people have migratory pain (pain that moves around the body).</p>
<p>Risk Factors of Fibromyalgia<br />
One study reported that 28% of the children of mothers with fibromyalgia also develop the disorder. Offspring who developed fibromyalgia were no more likely to have psychological disorders than those who did not.</p>
<p>Primary fibromyalgia is the most common type. Many experts believe that fibromyalgia is not a disease but rather a chronic pain condition brought on by several abnormal body responses to stress. Physical injuries, emotional trauma, or viral infections such as Epstein-Barr may be triggers of the disorder, but none have proven to be a cause of primary fibromyalgia.It is believed that individuals with FM may have low levels of certain chemicals in the brain such as serotonin and norepinephrine. Low levels of these brain chemicals can cause depression and contribute to the pain and fatigue experienced in FM.</p>
<p>How is it treated?<br />
Analgesics or &#8220;pain relievers&#8221; interact with receptors in the body to stop the sensation of pain from various sources. Analgesic drugs vary in strength and addiction potential from over-the-counter Tylenol to stronger prescription medications such as propoxyphene/acetaminophen (Darvocet) and tramadol (Ultram).</p>
<p>Lyrica: This is the first drug approved by the FDA specifically for the treatment of fibromyalgia. While this is a step forward, it is no cure. Lyrica has been shown to cut pain levels in half, but only in 30% of the people who took it.</p>
<p>Exercise. The emphasis is often on muscle conditioning and programs to improve aerobic fitness (such as swimming, cycling, walking and stationary cross-country ski machines) as well as physical therapy. Patients should be told that exercise is safe and effective. After an initial training period, the exercise regimen chosen should be done daily for 30 to 40 minutes.</p>

<p>What is <a  href='http://curesfibromyalgia.com/wp-content/plugins/wp-affiliate-pro.php?id=2' target="_blank">fibromyalgia</a>? It&#8217;s an arthritis like condition that causes diffuse muscle pains and excessive fatigue in the sufferer. Surprisingly enough, it affects as many as 2-3% of the general population, primarily middle aged women, and is an important cause of missed days from work and loss of productivity. Previously, there hasn&#8217;t been an FDA approved <a  href='http://curesfibromyalgia.com/wp-content/plugins/wp-affiliate-pro.php?id=2' target="_blank">fibromyalgia</a> medication available to treat this condition. Prior treatments for fibromyalgia, focusing on reducing levels of pain through pain management, have been relatively ineffective in most patients. Plus, new treatments have been difficult to develop as the cause of this perplexing disease is still unknown.</p>
<p>Fortunately, the FDA just announced the approval of a new fibromyalgia medication called Lyrica to treat the symptoms of this condition.. Interestingly enough, this is not a new drug to the medical community.It&#8217;s been used to treat a variety of other conditions unrelated to fibromyalgia including pain associated with diabetic neuropathy, pain associated with shingles and treatment of partial seizures. This would be the drug&#8217;s first approval specifically for the treatment of fibromyalgia symptoms.</p>
<p>Before approving this drug, the FDA looked at the results of randomized, controlled, double-blind studies on over 1800 people before giving their support and approval for the use of Lyrica in fibromyalgia patients. This fibromyalgia medication was shown to decrease pain levels in patients affected with the condition as well as improve their ability to carry out their daily activities. Reduction in pain levels were seen as early as the first week of treatment and were associated with significant improvement in sleep patterns as well as improvement in levels of fatigue experienced during the daytime hours. Adverse effects included difficulty concentrating, dizziness, and daytime sleepiness. There were also some reports of visual changes, weight gain, and dry mouth in patients enrolled in the studies. The side effects experienced by the patients in the studies appeared to be dose dependent.</p>
<p>Could this be the answer that fibromyalgia sufferers have been looking for? It&#8217;s too early to tell, but the preliminary test results look optimistic. If you or someone you know suffers from this condition, you may want to ask your family doctor to tell you more about this newly approved fibromyalgia medication and ask him or her if it would be right for you.</p>
